The UK Marine Accident Investigation Branch (MAIB) has published its findings on the October 24, 2023, collision between the bulk carrier Polesie and the general cargo ship Verity, which sank, resulting in five fatalities. The incident occurred in the German Bight traffic separation scheme, where both vessels’ watchkeepers accepted dangerously close passing distances. Their attempts to avoid the collision were neither timely nor clearly communicated, and the use of Very High Frequency (VHF) radio could have facilitated an earlier resolution.
The investigation revealed that the German Bight vessel traffic services (VTS) intervened too late, missing the chance to guide the vessels effectively. The damage to Verity led to its rapid sinking, leaving the crew insufficient time to don safety gear. Key safety issues identified include the necessity for timely and visible actions to prevent collisions, maintaining proper lookout, and the critical role of VTS operators in ensuring maritime safety.
Recommendations have been made for both shipping companies to reinforce compliance with collision prevention regulations and for the German Directorate General for Waterways and Shipping to enhance VHF communication protocols. Chief Inspector Andrew Moll emphasized that the tragic loss was avoidable and highlighted the importance of timely interventions and effective communication in maritime operations.
