MAIB Report Reveals Watchkeeping Failures Led to Verity Ship Collision

The Marine Accident Investigation Branch (MAIB) reported that the cargo vessel Verity sank shortly after colliding with the bulk carrier Polesie in the German Bight on October 24, 2023. The investigation, detailed in an extensive report, identified critical watchkeeping failures on both vessels and a delayed intervention from German vessel traffic services (VTS) as key factors contributing to the tragedy. Verity, managed by Faversham Ships Ltd since 2008, was transporting 3,262 tonnes of steel coils from Bremen to Immingham at the time of the incident.

The collision occurred in darkness, approximately 12 nautical miles southwest of Heligoland, with both ships detecting each other early on radar and AIS. However, neither crew took appropriate action to avoid the impending collision. Verity’s officer of the watch intended to maintain course, while Polesie’s chief officer made ineffective autopilot adjustments, ultimately leading to a collision at around 12 knots. This resulted in significant flooding and the rapid sinking of Verity within minutes, leaving little time for crew members to react.

The report highlighted significant shortcomings in adhering to collision regulations and noted the absence of additional lookouts on both bridges during the night. The VTS’s late warning about the close proximity of the vessels was also criticized, as it came when they were already dangerously close. The MAIB emphasized that both vessels must enhance their adherence to safety protocols, particularly during periods of limited visibility, to prevent such tragedies in the future.
