A recent Transport Accident Investigation Commission (TAIC) publication revealed a near-disaster involving the Achilles Bulker, a bulk carrier that faced a serious crisis while departing from a New Zealand port. On July 24, 2023, as the vessel navigated Tauranga’s waters, it unexpectedly veered toward the port. The helm detached entirely, causing the ship to drift dangerously close to shallow waters.

Thanks to the swift actions of the crew and pilots who deployed the anchor, a catastrophic grounding was narrowly avoided. The TAIC investigation revealed that a key component, the rowing pin, had gone missing. This part is essential for stabilizing the helm; its absence allowed excessive movement in the steering system.

The investigation pointed to improper maintenance carried out in 2021, during which the pintle assembly was removed and incorrectly reinstalled. This faulty reinstallation posed a significant risk, as the securing nut had dislodged, leading to eventual structural fatigue and failure.

The TAIC emphasized the critical importance of components like pintles for maritime safety. They recommended that shipyards implement strict quality assurance protocols and urged Maritime NZ to collaborate with international bodies to enhance global maintenance standards. This incident underscores the necessity of being prepared for unexpected challenges in maritime navigation.
