ExxonMobil Canada Properties (EMCP) recently reported a near miss event that took place on August 7, 2024, during a crew change between the survey vessel Amazon Conqueror and offshore supply vessel Victory G off Canada’s East Coast. Operations were temporarily halted due to concerns that the gangway used for personnel transportation was approaching its operating limits. Subsequently, the gangway detached from the landing platform on the Victory G, leaving it suspended between the two vessels. Fortunately, no injuries were reported as access to the gangway had been restricted prior to the incident. The gangway has been taken out of service for repairs and recertification, as it had the potential for a fatal outcome.

EMCP has launched an investigation to determine the root cause of the incident, with oversight from the Canada-Newfoundland and Labrador Offshore Petroleum Board (C-NLOPB). The safety of personnel remains a top priority for EMCP, and measures are being taken to prevent similar incidents in the future. The incident serves as a reminder of the importance of strict adherence to safety protocols and procedures in offshore operations. It also highlights the need for ongoing vigilance and maintenance to ensure the safety of workers in the offshore industry. The findings of the investigation will inform any necessary improvements to prevent such incidents from occurring again.
