radar lookout failure

Swedish authorities have charged the captain and pilot of the cargo ship Misje Verde following a collision near Tjörn that resulted in two deaths. Both face charges of negligence, as an investigation revealed they failed to maintain proper lookout, despite the pleasure boat being visible on radar prior to the incident.
