The Transport Accident Investigation Commission (TAIC) of New Zealand has released a report on an incident involving a crew member on the bulk carrier Poavosa Brave on June 23, 2023. The crew member was seriously injured when an on-board crane struck him while hoisting stanchions for securing logs on the main deck. The accident occurred because the safety-critical team leader, the bosun, did unauthorized work without informing responsible officers, bypassing safety protocols.
The master had initially instructed the bosun not to use the ship’s cranes due to unsuitable weather and sea conditions. However, the bosun decided to train the deck crew in using the crane to hoist stanchions without informing the master or chief officer of the change in plan. When the chief officer realized the unsafe work being carried out, he intervened, but communication breakdowns led to the accident.
TAIC emphasized the importance of communication in preventing accidents, stating that the bosun should have informed the chief officer and master of the new work plan involving the crane. Had there been proper communication and coordination among the crew members and officers, the accident could have been avoided. It is crucial for all personnel on board to speak up and raise safety concerns to prevent such incidents in the future.
