The UK Marine Accident Investigation Branch (MAIB) has released a report detailing the investigation into a severe engine failure and fire aboard the site investigation vessel Kommandor Susan on January 25, 2025, in the Firth of Forth, Scotland. The incident occurred during sea trials when a diesel generator engine failed catastrophically, leading to a fire in the engine room and a total power blackout.
The crew acted swiftly to extinguish the fire and ensure their safety. However, the vessel lost propulsion and began drifting eastward, with attempts to deploy anchors failing due to the lack of electrical power for the winches. Eventually, Kommandor Susan was returned to Leith harbor with limited propulsion restored.
Key safety issues identified included the use of non-genuine engine components during a 2019 overhaul, the approval of extended service intervals based on the assumption of using genuine parts, and insufficient oversight by the vessel owner, which allowed these components to be installed. Although no formal recommendations were made, the Chief Inspector of Marine Accidents has communicated to the vessel’s previous owner, Hays Ships Limited, the necessity for a structured supervision system and enhanced monitoring of critical maintenance activities.
