Fatal Lifeboat Accident on Malaysian FSO Claims Three Lives

On Saturday, a tragic incident occurred off the east coast of peninsular Malaysia, resulting in the deaths of three men and injuries to another during maintenance work on a lifeboat. The four contractors were performing a maintenance check aboard the Sepat FSO unit when a mishap involving a parted wire rope or hook system led to the lifeboat falling into the water. Local reports indicate that the contractors were evacuated to a hospital in Kuala Terengganu, where three were pronounced dead upon arrival, while the fourth remains under close observation.

The deceased individuals have been identified as Ahmad Fikri Zakaria, 38; Nik Muhammad Hafifi Asri Ab Majid, 37; and Muhammad Faezuan Hakim Mohammad Bustamam, 28. Bustamam’s wife shared that he had communicated with her earlier that day, expressing his eagerness to return home for a vacation. Initial reports suggested the incident was part of a lifeboat drill, but Petronas later clarified that it was indeed during maintenance work.

Lifeboats are particularly hazardous due to their height above the waterline, making mechanical failures potentially deadly. Historical data indicates that lifeboat drill failures have contributed significantly to seafarer fatalities over the years. Although safety measures have been implemented, such as restricting crew access during drills, the need for routine maintenance continues to pose risks.
