Among the highlighted cases, the Dali incident involved a containership that struck Baltimore’s Francis Scott Key Bridge after losing electrical power, resulting in the bridge’s collapse and the deaths of six construction workers. This investigation led to significant safety recommendations, particularly regarding the integrity of bridges over navigable waterways.
Another critical case covered is the Titan submersible disaster in 2023, where all five individuals onboard perished while exploring the Titanic wreck. Investigators identified serious flaws in the engineering and testing of the vessel, along with inadequate emergency response protocols.
The report also details the 2023 fire aboard the Grande Costa D’Avorio at Port Newark, which resulted in the deaths of two firefighters. The findings revealed significant gaps in firefighting procedures, resulting in new safety recommendations.
